Dear Clients: We are dedicated to the promotion and restoration of health. We understand optimal health to be a state of harmony and balance of the body, mind and spirit. Through education and personalized care we encourage and assist others on their journey back to health. We work as a team because we know that together we can be most effective. In our experience lasting improvement in a person’s health requires his/her increased focus and dedication. If you are somewhat overwhelmed by what we are telling you, simply think about your most satisfying and worthwhile accomplishments to date in your life. Then ask yourself if commitment was involved. Of course it was and the rules are no different with respect to our health challenges. So just focus on the rewards and remember that all journeys, great or small, start with a single step. Whether this is your first step or one of the many you have already taken on your path to a higher level of wellness, may you find a comfortable place. As part of our commitment to a high quality holistic health care, we have established the following policies. 1. Our aim is to teach the Client how to take charge of his/her own health and to maximize the healing process by working as a team. This means that we may share information about your progress with others who can provide assistance. However, we will also keep confidential anything that you specifically request us to do so. 2. We require your signed permission to communicate information about your progress to any health care professional outside our clinic and ask you to so indicate in the section on the back. 3. Your file is your property and you have access to it on request. 4. Fees are due at the time of your visit unless otherwise established. 5. Fees are charged for appointments broken without 24 hours notice without sufficient reason. 6. We appreciate your feedback about your experience here with us in order to improve the quality of our service. We welcome your comments at any time.
